For all your "criminal info" anything above 10 years is excluded per application requirements.
However, for your driving record I would include anything you can remember.
Apply for NDR and see what is indicated. Local DMV will only indicate state violations, NDR will include all states.
If it doesn't show up, it's your call, how much or how little you want to put on your application.
