This has no time expiration and should be disclosed as requested. The word "ever" pretty much sums it all up.
Agree, I forgot about addendum section. Ultimately individual pilot disclose information to his or her best knowledge and ability interpreting the question, personally I believe applicants should be as honest as possible, and explain any adverse situation during an interview.
