Never, NEVER withhold information for a prospective employer. Don't assume they'll never find out about something just because you don't tell them. They will eventually find out, you will be in a heap of trouble, and your career will suffer, because then you'll have to explain to everyone why you got into even more trouble by not disclosing.
Be 100% candid and upfront with all your failures. Owe up to them, explain them, state clearly where you were at fault, what you learned, and don't make excuses.
