PRIA is pilot records improvement act...these forms were the FAA forms included in the big app you had to send to JB before the interview. For military folks, I assume they are sent to your branch to obtain any flying related info in your record. For us civilians, it includes training info, checkrides, drug/alcohol test info, current position/quals, etc...
