If you have not identified your issues then do so. Correct them and go back. Don't get defensive about it. If I were an employer looking at you as a prospective candidate, I would ask myself: "If this guy is flying the airplane that crashes and kills 63-65 people, how is it going to look on the 6 O'clock news?" ...."PSA pilot with multiple training failures kills 63-65 people!" If PSA has invited you back, you're fortunate.
