What about calling the US Airways Employee Travel line at 800-325-9999?

You can just say "No ID" and then "Another Airline ID", that may connect you with someone who can help. Have you tried calling them?

Maybe they could at least make the jumpseat listing for you so that you have a record locator to work with? I don't know if you could pay the taxes and any other fees using a credit card while on the same phone call. The other option would be that once you have created a record locator for the return trip, you should be able to pay the fees for the return flight at the ticket counter in the USA before departing on your outbound leg.

Other than the time constraints associated with last minute travel, I don't know why you'd want to list for the JS as an OAL pilot. Since you can't physically occupy the FDJ on an international flight, your priority for a seat in the cabin puts you below all the other non-revs by only having a JS listing. Your seating priority would be better off having a non-rev listing as US Express affiliate carrier, or even if you purchased the ZED ticket.
