Just did this a few weeks ago. As previously mentioned:
1. Make an appointment with the FSDO
2. Show up with a paper 8710 filled out. The online system will not give the inspector the option to remove the 61.167(b) restriction.
3. Show up with your logbook up-to-date. My inspector wanted to see my paper log books. Not sure if electronic is acceptable.
4. Show up with your cert, medical, passport, etc.
5. Hope you get an inspector that actually knows how to do this.

Should take no more than an hour or two to issue you a temp certificate. You can also do this same process with a DPE but of course they will charge you, so I've heard, $50 or so.
