You might be able to call the HR contact on the announcement which you applied under and be able to find out some details about how you fell out of the process. Having just dealt a little with HR recently - I was impressed with the effort they went through to tell a person if something was wrong with the application/package.
Worth a try even if you don't learn anything new. You could also ask the question about reapplying under the same announcement.
I was about to call them yesterday and logged on to the USAJOBS site to get the phone number. All of a sudden, I noticed that my status has changed from Not Hired to Referred. I was placed on two referral lists, one at the GS-11 level and another at the GS-12 level. Now its time to wait for the phone call.
