I was a FAIP and gave a LOT of checkrides. I was not a SEFE and I had zero perspective. It abhors me now to know that guys might not get hired because I busted them for scaring the bejesus out of me on a turning rejoin.
Do what you want, but IMO UPT busts = crap under the noise level.
For the Pilotcredentials.com airlines, they don't really ask about pilot training failures. SWA, specifically, says to list failures but
DO NOT list pilot training phase check failures.
On airlineapps.com, they do say to list failures, including pilot training phase checks.
I'm going through the app process right now, so I'm kinda current on this.
