If the schedule deadhead is first class, that doesn't mean you can automatically get a first class deviation deadhead. You just get a larger bank because of the first class scheduled deadhead.
The upgrade form is for your deviation deadhead ticket when it only qualifies for coach but you want to fly in first class. You pay the difference out of your own pocket.

My scheduled DH was over 5 hours and was booked in first, but my deviation would be under 5 Hours. So in order to be allowed to book in first without filling out the form my duty day from revenue leg show time to end of my deviation leg needs to be 11.5 hours with out an intervening 8 hour rest period.
