The way to think about it, if you are deviating for a MEM to HNL deadhead you would likely be authorized a higher class of service, unless your actual deviation ticket is from Kona to HNL.

If the schedule deadhead is first class, that doesn't mean you can automatically get a first class deviation deadhead. You just get a larger bank because of the first class scheduled deadhead.

The upgrade form is for your deviation deadhead ticket when it only qualifies for coach but you want to fly in first class. You pay the difference out of your own pocket.
Thank you, your post clarified quite a bit.

My scheduled DH was over 5 hours and was booked in first, but my deviation would be under 5 Hours. So in order to be allowed to book in first without filling out the form my duty day from revenue leg show time to end of my deviation leg needs to be 11.5 hours with out an intervening 8 hour rest period.

Thanks. The more I looked into that the more I realized that had to be the case.

Or book a multi-leg deadhead that has 5:01 of block time regardless of crew rest or duty day

If you are deviating on an international leg it needs to be over 2:30 only to qualify for business/first.

Small point, but important. Your duty day needs to be greater than 11.5 hours. If it is 11+30, you don't qualify for first class, 11+31 you do. Also, 11.5 and less you don't just have to fill out the form, you have to pay the difference between the coach ticket and first class ticket out of your own pocket whether you have left over bank money or not.

A couple of questions:

1) When does duty day start? Showtime in the lobby? One hour prior to flight?

2) When does duty day end? When the deadhead arrives at your home airport?

I have not tried this, but I will in the future.

It's Section 12.B of the contract. Generally begins 1 hour prior to scheduled block out and ends 30 minutes after your deadhead (scheduled or deviation depending on which one you actually flew) is scheduled to block in.

And, if it's an international deadhead flight (or intercontinental?), your duty time begins two hours prior to departure.
