Would either of these things make it difficult to pass a 1st class medical exam?

I'm not a doctor but I've held a 1st class medical for decades. The FAA is a lot less strict than the military. The AME you visit for your medical exam won't get your DOD file. As long as you meet all of the requirements the day of your 1st class exam, you will pass. Follow the directions on the application carefully and disclose what you need to but less is more. The FAA worries most about chronic conditions not acute temporary problems that have resolved.
