Where do pilots usually position to get to the airplane, are there typical cities ?

Do pilots and flight attendant crews usually stay together for the entire stretch ? If Omni provides home basing does the pilot keep airline miles or hotel points, and is there a provision for business class travel if international positioning is required ?
