One of the ways we learn is through understanding the tragic mistakes and errors of those who died, were injured, or had near misses. No one wants to dance on a grave. But, all fields of endeavor can be made safer in the future…

Reminds me of the 1700s and early 1800s railroad engines. They had regular fatalities. The fireman kept shoveling coal to make the fire hotter, to increase the steam pressure, to make it go faster. Push the envelope, performance at the sacrifice of safety. Inevitably the steam boilers burst, killing people. As a result, boiler design and operating codes were established. Pressure relief valve were installed, and then it was prohibited from tieing them down (override of a safety system). Fatalities dropped.
