i only know of 4 pilots who washed out of horizon training.

1 failed a drug test on IOE

2 were old guy who were given 6+ additional sim sessions and they admitted they couldn’t make required callouts / hand fly approaches.

1 was a young pilot who was given lots of additional training and had panic attacks in the sim, he failed his MV twice.

they were all in Q400 training too
