Nope, you bid it. The rates are used for grandstanding but also a lot of people choose it for the pay I'm sure. I would also assume that those rates include FDP which will fluctuate. Choosing to work 18 days a month sounds insane to me but to each their own...
This is also the payscale for their biggest aircraft. Good luck holding Global CA anytime soon lol. So largest aircraft on the highest working schedule. This isn't so much as impressive as it is, the absolute cap you can make unless you throw extending in, which believe it or not, many people are crazy enough to do.
