Well, that’s how most accidents happen. It’s not just one thing. It’s a perfect storm, a chain of errors. Break one link of the chain and there is no accident. There are a lot of question marks. I actually have seen the A320 FCU dial a different altitude all by itself once.
That is how accidents happen, but the swiss cheese holes or perfect storm are usually a bunch of fairly common errors/factors that all happen line up to make a one in a million event. It’s not multiple one in a million factors lining up to cause a one in a trillion or whatever event.
