Due to scheduling limitations with doctors, I have gotten notes AFTER I was sick. The note stated something like, "Patient said they had an ear infection." It's not a courtroom. The doctor does not need to prove exactly what is wrong with you.
This is true but the doctor has discretion about how much they are willing to communicate to a third party on their (3rd party) form. Most will write whatever generalized statement you ask them to write but they can refuse.
