All of it is the company's fault:
1. They're self insured.
2. They hire the administrator.
3. They set the policies and coverages.
...uhc/umr is just the middleman paid to obfuscate the issue and keep the company's halo intact with employee groups and the media (think the various best places to work lists and noncon anti union koolaid) while fighting the good fight on the ground against employees at every turn to absolutely minimize healthcare costs to the company.
