I completely agree with you, it's hard to imagine a chain of events that would cause an experienced crew to overfly their destination. It could be a career ending mistake, or there could be more to it...
They could get terminated, but they would probably get reinstated. Remember, USAir just reinstated the pilot that discharged his weapon in the cockpit while
disregarding SOP's, so if a pilot can be forgiven for
that, it seems that a pilot can be forgiven for just about anything these days.
