Yes. EVERY job that you have flown and received a paycheck for you need to fill out a PRIA form. Whether or not the flight school sends any info back is kind of up to them.

If you have questions about how exactly to fill out the paperwork, call the HR contact that you have PRIOR to the interview. DON'T SHOW UP WITH INCOMPLETE PAPERWORK. At least when I was working for pilot recruitment at Atlantic Coast, that was pretty much a no-go/elimination item.
