The NetJets app package has 3 FAA forms: 8060-10, 8060-11, and 8060-12. The 8060-10 is simple enough and can easily be applied as it is a request to the FAA for our records with them.

The other two forms are meant for our current employers, in my case the USAF.
The -11 is a request to release records under 49 USC 44703. Would the Air Force know what this means? If yes, who should the request be sent too? flight records?
The -12 is a request about drug testing under the Dept of Trans. Does the DoD share our drug tests with the DoT? I am pretty sure these tests results are not kept in med. records, if they are kept at all when they don't come back positive.

Has anybody completed these forms for any app?
