USAir CRM video - any info? circa 2001?
I was cleaning out the CRM library - gotta love those VHS tapes from the 80's - anyway, have a video that I'm hoping someone can give me background info on.
Produced by USAir - guessing 2001, since that's the copyright date. No other info. Runs 21 minutes and begins with a mechanic pulling the CB for the igniter on the rgt engine. Crew misses it, engine torches, non-rev FA commands an evac in the back. Pax jumping down the slide with right & left engines running and right perceived to be on fire. Recreated by actors but has the actual crew doing interviews (CA,FO, & three FA's).
Obviously a CRM video but can anybody give me background on the actual event, report number, etc.?

I don't know this for certain (I'm not familiar with the particular video you cite), but our training department has been known to take actual events where CRM/TEM procedures worked as they should and produce "what if" videos about the things that could have gone wrong if those procedures hadn't worked. This *could* be something like that. (We had one like that with a 737 with a spoiler panel(s) deployed ... a deadheading pilot told the FAs, who told the pilots, who returned to the gate before takeoff ... the training video, though, started with a mock news report of the aircraft off the end of the runway on departure and folks being rushed to the hospital ... the game was up when you recognized the TV reporter was played by a Senior Check Airman.)
Over the years, the training department at US has produced a LOT of material, much of which has made its way in to the training programs of numerous other carriers. Some of it is based on actual events, and other stuff is, of course, dramatized to achieve some instructional goal.

I remember that one when I was there in 2001. It was pretty old then, maybe it was just the a/v equipment!, don't know when it was made.

Venetian, thanks for the info. This one re-creates the event, then interviews the actual crew who based on body language, tone, etc. is understandably not happy to be there.
I inherited CRM stuff ftom all over the spectrum and no one ever cataloged or provided background info on much of the material.
I'm former airline, actually at a 142 school now and we've got a bunch of stuff from various carriers. Including stuff that they told us they'd sue us if we ever used it (thanks, AA by the way).
I'm trying to get a handle what's proprietery and what's ok to use. We create our own stuff as well.

Maintenance had been doing some work and accidentally left a cb out. Somehow the engine torched on start, or restart - I'm not sure of the event. Yeah, the pilots should have caught the breaker, but in practice many cb's are difficult to see. I could fault the aft f/a's for not stopping the miscreant, but the latter was in uniform which would generate some confusion at the very least.

This is consistent with what the video shows. Did not have the info about Tampa though. Thanks for the comments. On to the next CRM video-titled "Mission Implausible." Now this one's really great....The captain leaves the FAs because they are late getting to the crew van!
