Domicile Info/fleet/carrier - Please help

Domicile Info/fleet/carrier - Please help
To all US Major Airline Pilots:
I am a graduate researcher currently analyzing a large number of flights in the United States between Jan 1 2005 and Dec 31 2010 (over 23 million!). Specifically, I am trying to incorporate pilot domicile information by aircraft type in my dataset, and while this is common information for many of you, it can be surprisingly difficult to find through other sources (especially for past time periods).
I would be EXTREMELY grateful for just a couple of minutes of your time if you could help me fill out the information below. Basically I am looking for the airport codes of domiciles for a given aircraft at each of the following 6 carriers:
Note that I am analyzing the full 6 years 2005-2010. If a certain domicile opened or closed sometime in this range, please provide the approximate month/year of the change (even just the year, anything is better than nothing).
Also, I am not looking at what are the domiciles today, only the 2005-2010 period is of interest.
