I commuted out of OAK for two years, the only requirement was I provide an airline ID and I was able to get an employee pass (my airline does not operate out of OAK, and I was commuting mostly on FDX/SWA.) $30/month is correct, renewable quarterly via mail. I'll look through my email and see if I still have any correspondence and PM if I find it.
