Default RAF Puma Crash

BBC NEWS | UK | England | North Yorkshire | Pilot criticised over Puma crash

Perhaps British coroners have a different role in mishap investigations there, but whether he was right or wrong about causal factors, doesn't this go totally against the safety process?

Any Brits on here that would care to shed some light?

Even here in the states, accident investigations run independent of safety investigations. They have nothing to do which each other, don't have to come to the same conclusions, and are used for different purposes. It is the accident investigation that is used for disciplinary actions.
