Correcting a past medical application
I have recently decided to get back into flying. My medical has expired and I am now applying to renew it.
I noticed that in my previous medical application in 2014 I did not disclose a surgery that I had in 2009. The surgery was called a thoracic sympathectomy. I have no idea why I didn't include it on my application. As far as I can tell it is not a disqualifying condition for holding a medical so it's not like there was even a reason to hide it. I must have simply forgotten and would now like to correct the record.
If I include it on my new application, how will the FAA respond to the fact that I didn't disclose it on my previous application? Will they even notice?

I would most definitely report it going forward. As to correcting the omission in the past, I would discuss with AOPA Medical, AMAS, or an AME before proceeding. If you're up front about it I can't imagine anyone will throw much a fuss, but work through a professional group who knows how to handle these situations. I guarantee you that you aren't the first or last to do this.

I'd discuss with some authority as whisperjet mentioned.
But as long as you report the omission before they find it, it was accidental, and you did not fly with a grounding condition I can't imagine they would be very concerned about it.

I spoke with someone from AOPA's Medical Resources line. This is for AOPA members but the woman I spoke with was still kind enough to give me a few minutes of her time and a few basic pointers. AOPA has really been a great group of people in all my experiences with them.
Anyway she said the FAA will likely not make a big deal about it since neither of the surgeries nor the underlying conditions sound like something I would of gotten grounded over. She suggested I note them on my next medical application and should also include a letter outlined that I am making a correction to my record and the details of that correction.
I typed up the letter and my appointment with the AME is this upcoming Monday. I'll let you all know how it turns out.

One thing I would do would be get the records from your treating physician describing the surgery, and that you would be cleared to resume duties, because chances are the FAA will want that anyway to put in your file .

The appointment with the AME went very well. After he reviewed my operative reports from both surgeries, he issued me a first class medical on the spot. I offered to give him the letter I wrote to the FAA outlining the changes I wanted to make to my medical record but he said it wasn't necessary. Great guy. I'm relieved.
