PDT has pretty good insurance as well. Family of 4+ is about $350/month for the PPO and covers almost anything. Low out of pocket maximums ($200), $10/20 co pays for vists to doctor and specialist respectively. I've been happy with it the past 8+ years and I've definitely used it.
Single and married no kids would pay much less than $350/month.
Dental and high deductible medical plans are also available.

Assuming AWAC still has their EPO plan, it's basically an HMO coverage level with BCBS but no deductible, low $15 co-pays. My wife had several surgeries under the plan and our out of pocket cost was $0. These were $45k-$50k surgeries. The plan cost was fairly expensive at something like $350 month but if you have health issues it's cheap in comparison.
