Pilot Qualification Error, Article Research
I am wondering whether anyone remembers an incident where a corporate jet was flown with neither pilot qualified on type due to a communication breakdown. It was discussed in the Transport Canada Aviation Safety Letter prior to the online archives and I can't find it. I am guessing it occurred in the 1990s.
The incident followed a pilot scheduling error, and each pilot believed himself to be the first officer- one thinking that the other had traded seats with him in mentorship. On landing they discovered the error, however after an uneventful first leg they decided to complete the trip.
Does anyone remember this incident with more detail? I need more keywords or a year to find the documents I need for reference in an article. Any help appreciated!
