Airline apps / Line check screw up

Hey guys, tried searching but couldn't find much on the specific topic.

Got pulled off line some time ago due to my Jepps being out a revision during a line check, had another one a week later and everything went back to normal.

Anyways, in the Addendums section there are about 3 questions in a row asking about being pulled off line, exactly they are as follows :

Have you ever been the subject of an evaluation board or been removed from flight status, downgraded, or failed any portion of flight training either civilian or military?

Have you ever failed any proficiency check, FAA check ride, or line check?

Have you ever been removed from flying status by an employer or lost your flight rating for any reason?

Do I have to select yes to all 3 questions? I know the line check question is a yes, with an explanation, but does that preclude me to selecting yes to the remaining 2 questions asking about being removed from flying status? I don't want to have to select "yes" any more than I have to.

Sorry for the mundane question, and appreciate any replies or insight anyone has.

My best advice: Get a copy of your PRIA paperwork.
Failed line checks are reportable. You will have a "U" if you actually failed it. It could just be a small thing your company handled in house. If that is the case, then I would answer "No" to all of those questions.

Pretty sure it ended up going down as a "U" as I remember getting the e-mail with line-check unsat, afterwards. So I know it will pop up on PRIA. I will double check, though. Assuming that being the case, though, just not sure if I should select yes 3 times, or just yes the one time for the line-check question, as being pulled off line was collateral damage from that event, and not a separate event.

Reading the three questions at face value and taking them literally, you have to answer yes to all three.

The downside is that the computer screening will probably move your app further down the list with three boxes checked vice just one.

The upside is that the bust was very minor, lesson learned to check revisions every time you go fly, end of story. Won't hurt you much at an interview (once you actually get called).

As to whether you want to argue semantics, and possibly try to explain at the interview why you DIDN'T check the other boxes...well, you've been around APC long enough to know the answer to that.

I would hesitate to put "Yes" to the third question.
You were pulled from a trip and given a second line check which you passed.
Being "Removed from Flight Status" typically involves paper work in which you have a "Status Change".
I would say "No" to that particular question. You still maintained flying status, you just had to update your Jepps.
I understand how frustrating it is to have ONE small incident feel like it's ruining your career.

Reading the questions, sounds like only #2 would require a "yes" as long as you didn't receive a mishap board, grounding notice, suspension of license etc.
