Airliner operations questions

Have a few questions for folks, as each one I think differs from carrier to carrier (or even pilot to pilot).
Thanks for taking the time to respond:
1) When do you engage the APU and disable the APU (for departure and arrival)?
Do you?
-Disable it after startup
-Disable it after takeoff
-Disable it after 10k
-Disable it after 18k
-Enable it when descending past 18k
-Enable it when descending past 10k
-Enable it after landing/taxi
-Never, APU costs too much for fuel. Just huffers and external power for us.
2) When do you enable/disable your landing lights?
-When passing 18k?
-When passing 10k?
-Right before landing
3) What cost-index do you use when flying?
-Domestic (1-3 hour)
-Domestic (Cross country / 5-6 hour)
-International (8-10 hour)
4) What altitude do you typically set acceleration thrust and push the nose a bit forward to speed up to 250?
-1000
-1500
-Other?

"Airliners" are operated as outlined in the air carrier FAA approved aircraft flight manual and operations manual.
