I have an interview coming up and getting the prep and paperwork taken care of. Question about the 8060-11 and 8060-12. I have two former airlines I worked for PSA and Frontier. On both the -11 and -12 I have duplicates both made out to PSA and one made out to Frontier which doesn't seem right. I also have forms made out to airline "N/A" on both sets of forms. Has anyone else had this happen? I sent an email off asking if it was ok with no response yet. Thanks!
