When filling out PRIA paperwork should you fill out forms for all previous employers, including small Part 91 operations, and part time jobs that don't really keep any documentation on you?

Yes. You are required to include every person or company who employed you as a pilot (except military). The employers are required to respond to the request with any records on flying performance (and certain discipline records) that they have. If they don't have any records (ie part 91 owner/operator) that is fine, but they still have to respond.
