PRIA Form Question

So, I'm filling out my PRIA forms for my first ever airline interview, and i'm a little perplexed on one part of the PRIA Form. The page is NH 1003 FO, and it states to list any places i've worked as a pilot under Part 121, 135, or Part 91 in the past 5 years. Also, the guidance from the company says "If you've never worked as a pilot put NONE at the top, and then sign/date the form. If you've been employed as a pilot, list the last 5 years and sign/date the form. If Self-Employed then document a letter of currency in everything (to summarize)" . That being said, I've only had 1 job as a pilot and that was as a CFI/CFII at a 141 school. So, that being said do I even put my current CFI job on this form or do I go with the "NONE" option?

this part only pertains to air carriers operating under part 121,135, and 91
These are the entities which must request PRIA records from previous employers (and FAA / NDR) when hiring a pilot.
Part 91 employers are not required to maintain pilot records, but if they do then they are required to respond to a PRIA request.

Look it up on the FAA website. That would fall under the info which the FAA will provide to an employer. I suspect they don't get reported, but can't say for sure.
Don't ever let anyone find out you failed an FAA written...since all the answers are available for review anything less than mid-high 90's will raise questions in the mind of an interviewer.
