Probably more accurate to say radio work across the US is abysmal. It’s very good just about everywhere else and unfortunately many of the US flyers who were improperly trained quickly identify themselves with one or two radio calls internationally. They can get by with CB style radio calls in the US, but elsewhere they embarrass themselves, their airline and create a potential hazard for the rest of us trying to do it properly and professionally.

ATC radioed to the flight, "Tiger 66, descend two four zero zero (2,400 feet), cleared for NDB approach runway 33." Captain Halpin, who interpreted it as "descend to four zero zero" replied with, "Okay, four zero zero" (meaning 400 feet (120 m) above sea level, which was 2,000 feet (610 m) too low). The Cockpit voice recorder also revealed several communication errors made by the flight crew prior to this miscommunication and a general casual nature of the Captain, who was the pilot-not-flying on this particular leg of the trip.
During the final approach, numerous clear warnings were given by the on-board Ground Proximity Warning System which were all ignored entirely by the crew, and the aircraft impacted a hillside 437 feet (133 m) above sea level, killing all four crew on board; two pilots, a flight engineer and an aircraft mechanic. The subsequent fire burned for two days.

) of this thread is to point out the slow normalization of deviance, in this case sloppy and unprofessional phraseology. It’s embarrassing when I hear an American flying OCONUS using “on the hold”, “on the go” and yes, O instead of 0.

Negative!!! Ignoring 7 or 8 PULL UPs from the GPWS and confirming with the RAD ALT took down the F/T 747.
