Not to be a contrarian, but once you earn a title you should be addressed by it. For instance, you don't call a Doctor "mister". A Master Chief Petty Officer in the Navy has earned the right to be addressed as "Master Chief." A ship Captain is entitled to be referred to as "Captain". So is an airline Captain.
Now, you probably don't want to carry your Regional title when you move on to the Majors, but when you earn the fourth stripe at your current employer, you earn the right to be addressed as "Captain".
In the case of a scab like Mr. Green, that theory might not apply.

"Captain" is a job title at a company. You earn the right to be referred to it at work. It doesn't carry with you to outside life or another job, unless you have an ego problem.

You never went through initial training? You've never had a check ride? Or gone through recurrent? Or had a line check?
Not only is the title earned, its the ONLY one that has to be re-earned on a regular basis.

Yes, I went through initial training and I now have a 757/767 type with no restrictions. I could be a captain, but I am a first officer because others got here first. When I make captain, it will be due to time. The title isn't merit based, therefore, it isn't "earned."
