Posted this in the SWA forum, I'm pretty sure this pilot is the 1st at a major airline to get their wavier and fly while on insulin:

He's a Captain at SWA so probably not. CNN did a story on him, after he was diagnosed he quit flying but stayed on as an instructor at SWA:
