What they generally do for pilot or applicants that are outside of the 121/HIMS umbrella is this.

In the regulations it states that you need to have proof of two years of abstinence before the issuance of a medical. That means documented AA meetings and probably random drug and alcohol tests. That is the standard route for a part 91 pilot.

In the airline world we have what we call HIMS. Look it up, there is a lot of data out there on it. We have in the past let GA pilots in to this program, but that would be up to the Flight Surgeon. This guy they forwarded you to in DC is the guy that makes all of the decisions. The psyche testing etc is standard for a special issuance medical in the 121 world. It is then followed up by weekly, monthly, quarterly and yearly visits to an array of people. In your case it is all out of pocket. It is expensive.
What I would do is find a AME that specializes in AODA medical re-certification. They are a conduit between most 121 applicants and the DC Flight Surgeon.
I know the 121 world stuff really well as I work in it, but the part 91 stuff is a world unto its own. No matter what their decision is, you did the correct thing by reporting all of this on your initial application for a medical certificate.
You many need to jump through a lot of hoops but if you are willing and realize that you may have not just an abuse diagnosis, but a dependence or addiction one, you may be successful in your endeavor. PM me with any questions you have. I may not be able to answer them, but I can try.

acl65pilot is right. You've got alot hoops to jump through. In addition, I hate to be the bearer of bad news however more than likely, Dr. Fred Tilton, Federal Air Surgeon, will rule against you in pursuit of your medical certification. My suggestion is that you contact Kathy Lincoln at the FAA Medical Appeals Office in DC at 202-267-8112. Ask her if she would spell out for you the step by step process to get a Special Issuance medical because that's the only way you are going to get one.

However, I do attend on average 2-3 AA meeting per week, along with Bible Study and other Spiritually based gatherings, such as worship service etc. It will be suggested that you start attending AA meetings on a regular basis, get a sponsor, work the steps, get involved. This statement will make much more sense later on. Trust me.

Now, once your I.O.P is complete, you will undergo another HIMS evaluation. All reports from your I.O.P., your 2nd HIMS eval and any other pertinent letters of support from a therapist, your parents and yourself, should accompany your new app for medical certification. The AME that completes your 2nd examination for the certification for the airman's medical wll forward all of this info to the Medical Appeals Office in OKC for approval. If they are unable to make a determination in your case, it will then be forwarded to Washington DC to be reviewed and a final decision rendered. My I.O.P. ends in March. I wll get my second HMS eval done in the first week of April. Once the results come back from the HIMS, I will go to a local AME and apply for medical certification again. He will then send everything off to the FAA.


One thing to keep in mind. Out of say 200,000 applicants, only 0.2% are out and out denied. That's only about 400. The reasons vary from the person not pursuing the issue any further after a first denial or it is determined that relapse (recitivism) is a problem.

My advice to you is not to pursue the unrestricted medical. YOU WILL LOSE THE APPEAL. Instead, meet and exceed the criteria that the FAA has set forth for the Special Issuance program and enjoy a healthy, meaningful and well deserved career. Btw, you can be enrolled in SI program from 1-5 years.

Thank you for this info it was extremely helpful. I have 6 years since an arrest with substances 3rd of my life. finally got my life together and remembered how much i loved flying.I started flying when i was 15 but i never got my medical. Fast forward to 43 and i am looking forward to getting my PPL and i have aspirations on CFI rating.
I am glad that there is an ave. to pursue but it is a bit daunting. I wish i had know some of this stuff about 6 years ago lol.

Get a lawyer. If you do it on your own there is a slim to none chance of obtaining a medical. Best of luck
