They will name top leaders who are fired (CO/XO/Senior Enlisted Advisor), but do not name lower-rank personal who get disciplined or fired. Names often get in the media anyway, but it's not because DoD released them.

Final report out. Pilot was performing an **** Hot break to landing, two landing assist systems, one of which was auto-throttle, other maintains AoA, were not engaged. Cant link it, but someone else should be able to. Found on news.usni.org

Actually... what they're not telling you is that the pilot *attempted* to arm the automation but the 5G microchips happened to do a burst upload at that moment and the computer didn't get the signal from the button due to the interference.
