Happened in Oslo last year:

The LCA/IRO had food poisoning, or some other similar ailment. They weren't on fire, didn't have a passenger near death in the back, a pregnant woman giving birth, etc.

I remember reading about that in AvWeek & SpaceTech mag. There was an airfield analysis done after the fact, and they were short on required runway by about 200', as I recall. If memory serves, they (KAL?) had a little help with a headwind. And the main gear did go thru a snow bank at the extended runway centerline. God watches over fools, too. And sometimes (as has been pointed out) those who are just plain tired/fatigued. Be careful out there....
