This happened in Anchorage a number of years ago. A Chinese carrier flying an A340 actually took off on a taxiway. I don't remember all the details, but the taxiway they used was shorter than the runway they were suppose to use. I think there was a tire print in the snow berm at the end of the taxiway.

The controller at the time noticed that it was taking off on the taxiway, but didn't alert the crew because he was worried that they wouldn't have enough distance to stop.

That could have been a horrible tragedy. Think of all the hottie Finnair flight attendants that could've been hurt!!! Our size 28 red dress tomatoes would make a taxiway takeoff extremely unsafe.

I remember reading about that in AvWeek & SpaceTech mag. There was an airfield analysis done after the fact, and they were short on required runway by about 200', as I recall. If memory serves, they (KAL?) had a little help with a headwind. And the main gear did go thru a snow bank at the extended runway centerline. God watches over fools, too. And sometimes (as has been pointed out) those who are just plain tired/fatigued. Be careful out there....
