This is the only correct answer.

A Reuters article mentions a miscommunication of the flap setting between PF and PM.
A CNN article says that the PM misheard the PF and selected flap 15 instead of flap 5.
I had to stop reading after the SimpleFlying article.

The interesting part to me is the pilot statements in the NTSB report. They took off at flap 20. At Acc Alt, the PF says that flaps 5 was called. The PM said what was heard was flap 15. The PF said that vmo/mmo moved towards the speed bug instead of moving further away. Now, no matter if the flaps went from 20 (200kts) to 15 (215kts) or 20 to 5 (235kts), the vmo/mmo would have moved away from the speed bug.

The Captain said he was focused on the flap setting and was looking at our EICAS waiting for a flap malfunction to appear and was unaware of our pitch attitude until I queried him. The Captain said he did pitch forward, but didn’t think he pitched forward enough to cause such a dramatic pitch change and concluded we must have hit windshear or a downdraft. Neither of us heard or witnessed the Predictive or Reactive windshear warnings but we did hear the GPWS cautions and warnings

At my last airline, we always used the intercom. Same equipment here, the majority of pilots act like keeping the intercom off will impress Jodie Foster. I find myself more times than I should having to ask a callout to be repeated because I can't understand what they just asked for. I don't know what the SOP is on the 777 but I can totally see this exact scenario happening on my fleet.

This is my biggest gripe about the 756 fleet. Literally no intercom to use. Loads of communication issues and I slowly get to lose my hearing.

This I completely agree with. EVERY aircraft on my current fleet has an intercom yet most don’t want to use it.

I have flown the 777 (not here) and know that intercom is an option, so some have it and some don’t.

Do we know if they were using it or not?
Maybe should be a new policy of intercom use below 10 or 18? Or an individual policy discussed in the briefing.

Would a full thrust T/O at flaps 20 on a 5 hour flight pitch out at 20 degrees? I hadn't heard they pitched down to minus 10 in their "recovery". That is very curious.
