Problem is it’s not my old company…I’m military and apparently you have to get PRIA for any company you drug tested with (according to HR). Said other company is my only backup I have another CJO with (class date after UPS), which is why I’m hesitant to reach out to them.
I kinda don’t want to shoot myself in the foot with them until I have my FJO at UPS first.

It is odd that UPS is making you do that. For there is no PRIA for said company. I would try to see if you could escalate it, however, being UPS I could understand your hesitation.
Here’s a link to a PRIA form.

Hopefully you can find it useful.

PRIA is only for when you were actually employed with a carrier. Therefore, said carried, should have not been listed as a previous employer. You did a pre-drug test exam for onboarding purposes. Even though it is required for the DOT, it’s not a part of PRIA until you are actually working for said carrier.

Yes, when conveying the offer over the phone HR typically says something along the lines of “don’t let the lack of a formal offer keep you from showing up to class. If we need to pull you out after class starts, we will”.
If they give you a class date and later decide they don’t want you to show up they should tell you.
