Also assuming upgrade was on the same plane you had SIC in, they will wonder (and maybe ask) why you didn't learn enough in the right seat before upgrade. Be prepared to answer that.
Other than that, not a career catastrophe, since you have no other training problems. You'll probably want some professional interview prep before you apply to majors.

Why did you not go back to the right seat, and try again later? That's what majors will want to know. Did the employer have an up or out policy?

Well now this is a completely different story.
Was the second attempt in the same aircraft as the first upgrade attempt?
Were you a DEC?
Did you fly as an FO in it or was it a new type?
Did you fail for the same reason as the first time?

Every operation hires “captains”; not to start, but as their future captains. Seeing you didn’t cut it twice will lead to lots of questions as to ability to be a captain, especially as it was during OE, not a rating check. A rating sim check has a time compression, emergency procedure element that makes a bust more understandable. In OE, you have more time, more control over events.

That should never be an issue at an interview. Some people get nervous when a negative aspect of their application is brought up during the interview. Just remember that the knew about that BEFORE they invited you for the interview. That means they don't see a problem with hiring you as long as you have acceptable answers to their questions. Don't get defensive or nervous.

By issue, I meant being questioned at interview, not his reaction to the question. I’d agree, if he gets called and the details are in the application, it’s obviously not disqualifying, but the interviewee’s reaction might be disqualifying.
