121 and 135 carriers are the only ones required to request PRIA info from previous employers. But the requests must go to all employers who employed you as a pilot.

Previous 121 and 135 employers will have records which they are required to supply in response to a PRIA request.

Part 91 employers are generally not required to keep any records...but if they do happen to keep records, they are required to respond with what they have. This includes flight schools who employed you, and they often do keep records (141 schools are required to keep records).

People will say many things that they believe are right... when in doubt go to the books; it (PRIA to Pt. 91 Entities) is not required, it is not discouraged either, they can't really ding you either way, but again, it's not required, if they want to require it they need to change the regulatory guidance.

I was at a regional carrier. During training in the sim, I was let go after failing a sim session. I was never given a clear answer why I was shown the door. I never took the 121 checkride or oral portion. I also never recieved a pay check. My question now is do I need to fill out a pria form for that carrier? Is there any reason for me to divuldge that information in a future 121 interview?

So what's the consensus on this one? If it asks for you to fill out a form for all "pilot" or "pilot instructor" positions, is this just referring to any Pt 121/135 flying, or pt 91 flight instructing as well? The places I have flight instructed at have (obviously) not kept "training records" for me ....

What about contract work? I paid for my own training. I was never really employed by the company (they were part 135) since I was paid on a day rate.

Understood, but what if all flight training as well as flight instruction given was done as part 61?

What it a flight school or you freelancing? I only had to turn it in for the MAJOR flight school I worked for.

It was an FBO that also offered some flight instruction services. All pt 61. I do plan on turning in information for the University I've instructed for, but not sure what to do about the 2 FBO's. I'll probably call HR this week and ask.

Just some additional clarification: If you are applying at a 135 or 121 carrier, you will need to fill out PRIA requests.

If you were an EMPLOYEE of a company, (IE you received a W-2 at the end of the year) and you flew during the course of your PRIMARY duties, (IE you were hired primarily to fly an aircraft or serve as a crewmember on an aircraft), you need to complete a PRIA form and submit it to the airline that you are interviewing with, regardless of the type of operation (61,91,135,121,141)

Whether or not the former employer completes and returns the form is out of your jurisdiction... some companies (flight schools, part 91 operators) aren't great about keeping records, especially beyond a few years.
