What, you don't play "true confessions" on your safety reports? They do make for entertaining reading sometimes, but I figure if you feel the need to spill your guts...get a dog.

"I woke up early and had a coffee with two creams and one sugar. The limo showed up late and the driver ran a red light. He felt it was yellow. We got the paper work and hurried to the aircraft. Preflight was normal and we blocked out on time, but I felt very rushed. After the F/O's outstanding takeoff I realized that he had forgot (insert trivial item here). I strongly feel that we should've caught that much earlier. The F/O seemed distracted and his attitude didn't appear to reflect the seriousness of this situation. In the interest of safety FedEx needs to add a three page checklist to ensure this never happens again. At the end of the flight during debrief, I realized I had lost my lucky pen, which I feel contributed to this incident."
As an aside, do you wonder what some of the reports that they don't publish must sound like?

That was a classic.
Remember the guy who didn't think that it was appropriate for the F/O to show up at show time? He wanted everyone to come in 30 minutes early.
Between reading safety reports and listening to 131.92 (the comedy channel) you should get quite a few laughs.

Really, why anyone would fill out a report because they were stupid, is well.... just plain stupid. After all, what's to be learned from someone telling us that he was distracted and had a runway incursion, or that he missed the correct taxiway. What are those guys thinking? Do they actually think that if they turn themselves in, that the company will somehow come to their aid. They had better think again, because it'll never happen. In fact, the company could take that information and start trending that (those) guys. Safety reports should be about just that: safety. Not stupidity. There's nothing to be learned from you telling me that you busted an altitude or forgot to pull it back to below 250K below 10. We all know that stuff. Tell me about the fact that when the control wheel is 5 degrees left of center, while the jet is in a 4 degree nose-low attitude, that actuating the speedbrakes, flips the jet over. Now that would be a good thing to know, don't you think? The dumb stuff, keep to yourself and if need be, go to confession.
As for the captain wanting the crew to show up at a different time, as opposed to the actual check-in time, well, according to the FOM, he has that right. Hopefully, he'd also have enough CRM skills to let everyone know in advance and have a good reason for doing so. I think it's much less important domestically, but overseas, in say, Seoul, he might be thinking of the very long drive and just wants to make sure they don't run late. Sometimes having been there and done that gives one a perspective that might not be shared or understood by guys who have not had those experiences.

I have stopped reading them. When they forward items like the NAV light that was lost in flight, and the previously mentioned worthless reports they become usless to me. Send out the ones that really matter, like MX issues so we can know what is going on wwith the fleet.
To me the system is broken and they are not worth my time to read them.

Funny thing, guys use Flight Safety Reports for everything. Notice the replies from Flight Safety telling them to file the appropriate report. I can only imagine how many they get- they don't publish all of them. And when you put in how great your f/o was? They don't know him, they don't care, and all that gets deleted in Flight Safety anyway. Your f/o won't be getting a BZ for it.

I think the implication from the Captain was that the other guy should show up ahead of him to get things done and the Captain could review. Well that is fine if the FO gets paid to show up a half hour early and starts his duty day a half hour before the Captain.
Now, if in the middle of a trip and the Captain decides that the entire crew should show up early, then so be it. But I don't think that was the message in the FSR that was being talked about.

My friend with the local gestapo, (KMEM FSDO), a former legacy carrier big jet driver can't believe we do this to ourselves. Yes, they do see them, but they are sanitized. His recommendation is to comply with the FOM, and nothing more. NASA is for the rest.
