I havent used it yet, but... It's with United Healthcare. 3 different plans. The top tier plan is a PPO and is ~$140 for a 4-week period for single. Gets expensive quick for spouse and dependents. Is 20% deductibles for most things. $10 copay for most prescriptions. Preventative care 100% covered. Single yearly deductible $340. Out-of-Pocket Maximum (Includes deductibles and all copays) $1500. (I couldnt tell you the difference).
Dental is $50 single yearly deductible. $8 per month out of paycheck.

For someone wanting to be in MSP with 0 121 time, if any Compass guys have insight on Compass as a better option than Endeavor PM me. Length of time to hold MSP, reserve, upgrade for a 0 hour 121 timer, stuff like that. Have a couple of CJO's with a decision to make. Grew up in MN looking to fly for Compass with the flow to the airline I want to work for one day, but I know things have changed since the sale and loss of all that. Also any Guard/Reserve pilots that fly here let me know how days off work and if it is not bad with Compass. Appreciate it in advance.
You need 2500TT and 1000 P121 to qualify as Compass captain. Word around Compass is lots of FO's are getting their time and becoming qualified for CA. HR is telling people on phone screening that a 1.5-2year upgrade seems accurate for your expectation. With that said, no one can tell how it'll shape up.
As for Guard, airlines cant discriminate for your service.

I'll maybe let someone in the know answer on MSP, but...SEA FO is more senior than MSP FO. And MSP CA is more senior than SEA CA. It'll take maybe 6 months to hold MSP as FO, in my opinion. But I am not MSP based.

Of course, in a month this could all change again. But this is what I have witnessed personally in the last 30 days.
As far as upgrade. 1.5-2 sounds right for zero 121 time for anyone hired in the last year.

Looks like the latest new hire class is split SEA/LAX. With the 4 bottom guys getting SEA.
