How does this happen during taxi? Return for emergencies? Was a pack on MEL then the the other one failed?

Ship numbers in this thread but I find it hard to believe any Captain would accept a aircraft with a pack inop with temps above 110.

Happens all the time on older aircraft like the 73N, 757 and 717. Some older planes, even with a working APU, have inadequate airflow on the ground for extended taxi in high temps. 717 was like a sauna in Summer time. Combine that with record heat in Vegas and an unexpectedly long taxi out and problems like this arise.
