Fatigue, disrupted habit patterns, mechanical failure in one case........ but mostly basic human error. One would think it's possible for a crew to focus for that few minutes and achieve perfection, but the results speak for themselves. It happens.
There is an easy solution to this. Let the ground personnel on the OUTSIDE of the aircraft open the door. It's no longer a money issue since they linked block-in with beacon off and not the door. So, why are we still doing the door dance? All the pax airlines operating 777s (and any other non-manual girt bar aircraft) figured this out decades ago and guess what?..... they don't blow slides.
Re-programming the in-message logic is saving an average of $3000-$4000 per day (right out of our pockets) in 777 crew pay across the fleet. Use some of those savings to train ground crew, re-print the manuals, put a couple of FCIFs out and call it good.
Or..... send a threatening email and probably come up with more layers of procedures to accomplish a simple task.
What's next? Have one or two people from the ground staff come in through the cockpit windows and help with the "doors" call and stand there watching while the two pilots involved work the doors.

I guess airing out your dirty laundry in a PUBLIC setting is cool. What next, domestic problems? Not sure how this post helps, but I’m thinking you haven’t thought this one through.

The bottom line is that we, as an airline, are being stupid. After 18 inadvertent slide deployments on one aircraft type, I seriously doubt we're keeping that fact secret. There are proven, simple solutions to stop this trend. Perhaps shining a spotlight on our failures will help those who perpetuate the procedures contributing to them make a better choice.

Not sure what "domestic problems" are in relation to aviation safety. I'm also pretty sure this was a highly "public" event wherever it occurred, especially if someone was injured.

I don’t disagree whatsoever with your points on the issue at hand. I just disagree with posting the dirty laundry in a public setting. It doesn’t help one bit and just comes across as standard complaining with no action. If you really wanted to make a difference, you could’ve chosen to give your suggestions to the company safety department or management, ALPA, or in a private forum like jetflyers where only FedEx pilots have access. But posting it here inevitably make us and the company look like a JV squad. Just saying, there are better and more effective places to talk about this and make a difference.
On a side note, I don’t know you from Adam, so the “domestic” comment was completely rhetorical. It was meant to highlight the next line to cross in acceptable posting behavior.

If you don't think the company reads this forum, you were born last night. I would think they find the ideas on here useful.
This will not stop the general public from shipping a single package, and most non pilots will not care about this subjects. Look at the other APC forums. This is tame.

You are completely missing the point.

What are you, the posting police? Must we respect your authority? Hows about we PM you what we want to post and you decide, does that work for you, officer?
Do other pilots read this stuff and actually think we’re JV?
Or do other pilots read this stuff and think we’re a bunch of tools?
Hey, are you on the airbus? I know you. Flew with you once. Once.
