On the application how are you guys listing 141 stage check failures. Is that a training failure or a checkride failure? I've got one end of course failure for my CFI that I have listed as a checkride failure, but was unsure of where to put a stage check failure that wasn't getting a certificate. My 141 school had self examining authority.

Training failure: Is if you washed out of training.
Checkride failure: If you failed a ride, PC. Anything you recieved a pink slip on that gets processed by the FAA. An EOC is a checkride depending on how the school processes the check. If the result one the EOC would be in the issueing of your CFI certificate then it is a checkride. This would go under checkride failures.
Important thing is what can they prove. If they can see it in the interview then declare it. If they can't see it then forget it, but I
